Our reason for putting appraisal in the third slot is instructional: judgment of single studies has to precede synthesis across them, and that ordering comes from how we teach doctoral writing. It is not a schedule we copied from a Walden document, since the university publishes neither its syllabi nor its course guides where an outside reader can reach them. What an appraisal rubric actually rewards
The first row wants the design named correctly, because everything downstream depends on it. A prospective cohort, a quasi experimental pre and post, a stepped wedge rollout and a single site quality improvement report answer different questions, and calling one by another name undermines the rest of the appraisal.
The second row looks for the threat you noticed. Every study has one: an unblinded outcome assessor, a secular trend running alongside the intervention, attrition concentrated in the sickest patients, or an outcome measured by the people delivering the change. Naming the specific threat beats reciting a generic limitations paragraph.
A third row grades applicability. Effect sizes travel badly across settings, so state whether the sample, the staffing model and the baseline rate resemble yours, and say plainly when they do not. Doctoral readers respect the appraisal that argues against its own convenience.

Randomization guards allocation, blinding guards assessment, a control site guards against background trends. State what the study bought with its design so the reader knows what it did not buy.

Hunt for the threat the authors left uncontrolled
Look for a co-occurring initiative, a staffing change, a seasonal pattern, or an outcome that improved because measurement improved. One specific threat, described concretely, satisfies the row that a paragraph of hedging does not.

Separate a significant result from a meaningful one
Report the size of the effect and the interval around it, then say what that size would mean on your unit. A statistically clean two percent shift can be clinically irrelevant, and the reverse happens too.

Hold the sample against your own population
Compare age, acuity, payer mix, staffing ratios and baseline performance. A study run where the baseline rate was triple yours cannot promise you the same room to improve.

Finish every appraisal with a single sentence stating what this study contributes to your project and how heavily it should count. Those sentences become the raw material for synthesis.

Five mistakes that cost points during appraisal
- Design taken from the abstract. Authors describe their own work generously, and a design misnamed at the top of a row poisons every judgment below it.
- Limitations copied from the paper. The authors' own limitations paragraph is a starting point, and a row asking for critical judgment expects the threat they were too close to see.
- Significance treated as importance. A p value reports how surprising a result is under a null assumption and says nothing about whether the effect would change a shift.
- Every study weighted equally. A single site report and a multi site controlled trial do not carry the same load, and a table that treats them alike cannot support synthesis.
- Applicability assumed. When a sample looks nothing like your population, saying so protects the project far better than quietly hoping no one checks.
